St. John's Photo Release Form for minors under age 18

Occasionally our children and youth will be photographed at church or during a church event. This release allows St. John's to use your child(ren)'s photo for inclusion in church related materials disseminated through our publications, website, and Social Media (names will never be publicized). One release form may be used per family.
